There is a rather obscure bug that causes some NPCs to simply ignore you when you are in werewolf form. They are not hostile, and sometimes even friendly. This was not intended by the developers, as all NPCs should attack when the hero transforms into a beast. Thanks to this mod for Skyrim Special Edition, NPCs will again start attacking the hero when transforming into a werewolf.
