To get the Star Ending in Cyberpunk 2077, you need to unlock it by completing specific side quests: "Riders on the Storm," "With a Little Help from My Friends," and "Queen of the Highway." Complete the "Riders on the Storm" mission.
Complete the "With a Little Help from My Friends" mission.
Complete the "Queen of the Highway" mission.
Call Panam.
Take the Omega blockers.
Return to your own body (choice).

Detailed Actions

1. Find Panam's number in your phone contacts. Call her. She will be willing to break into Arasaka's office with you.
2. To obtain the final Tarot cards, ask Misty to read Tarot for you.
3. Panam will take you to the heart of the Aldecaldos.
4. You discuss options for infiltrating Arasaka Tower.
5. Dakota is willing to talk to Alt to get some help at Arasaka.
6. Talk to Mitch. Listen to his stories about the Basilisk.
7. Sol accepts us into the clan, and V receives a jacket.
8. After the night, go explore the construction site. Then make your way into the Basilisk.
9. You'll have to fight your way through the tunnel.
10. When you reach the tunnel itself, find Mitch near the storm machine. Ask him to return to the clan in the Basilisk.
11. Clear the path to the tower and eventually infiltrate it with Panam and Saul.
12. Sneak past the guards with Johnny to reach Alt.
13. Alt will fight the security while you head to the Mikoshi.
14. Final battle with Adam Smasher (Johnny's killer).

During the battle, Saul will die, and you, Alt, and Johnny will enter the cyberspace. Here, you will face two choices:

• Return to Earth in your own body and live out your last days. During the conversation with Johnny on the balcony, choose the following responses: "Call Panam," then "I need to do something quickly today," "Ask Panam for help," and finally, accept the Omega blockers and decide to leave with the Nomads. If you choose this option, V will leave Night City with the Nomads and meet their death somewhere out there. If you had a romance with Judy, she will leave with you.
• Give your own body to Silverhand and exist in cyberspace as part of Alt. If you choose this option, Johnny will return to Earth and live by his own rules. Soon, he will also leave Night City with hopes of making his life better.
