The fourth and final puzzle "Labyrinth" in RE8 is located at the Geisenberg Factory. You need to find the "Iron Horse" sphere form, create it using the foundry device, and then solve the puzzle.

Where to find the sphere form
You can find it at the factory, floor B2.
After fighting the Lycans, you will climb up the grinder shaft. Before you enter the ventilation duct where Ethan almost gets sucked into the spinning pipe, there will be a secret room to the right of you. Push the cart (see photo) and there will be a small ventilation opening hidden behind it that you can crawl through.
When you get to the other side, the sphere form will be inside the suitcase.
